Student Emergency Form

This form is needed anytime you have had a change in your status – like moving, new email address, new phone number, etc. Please hand it in at the front office.

Meal Application

This application is for school meal benefits for qualifying households. Parents and guardians can submit a meal application online. The application will be processed within 24 hours, and you will not have to fill out a paper application and mail it or hand it over to the front office.

Beyond the Bell (YS)

This form is for 2nd – 5th grade students staying on campus after school. Please fill out this form and give it to the office before your child stays without a parent after school.
